Admissions FAQ

C4PW’s Intake Coordinator conducts an initial screening (in person, over the phone, or online) to determine if the potential client meets the admissions criteria. If the potential client meets the criteria, an intake appointment is scheduled to collect information, explain our programs, and complete the admissions process and orientation.

Yes, C4PW is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF).

C4PW understands that due to the nature of our clients’ substance use and/or co-occurring disorders they may have pending legal issues. Therefore, these issues are addressed through case management and treatment services.  Our knowledgeable and compassionate staff will support and provide any assistance needed.

C4PW will provide transportation to clients who need to get to our center and return home. 

Clinical FAQ

 Cognitive Behavioral Therapy, Solution Focused and Goal-Oriented Therapy, Motivational Interviewing, Meditation and Mindfulness, Social-Structure Social Learning Theory, Mindfulness Therapy, Prosocial Skills Education, Stress Management, Stages of Change Counseling , Psychoeducation, Life Skills Education and Relaxation Therapy.

C4PW maintains client’s personal and treatment information in strict confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPPA) and all applicable local and federal laws.

Generally, clients placed in the Intensive Outpatient Program will receive individual therapy once weekly and clients in the Partial Hospitalization Program will receive individual therapy twice weekly.  However, each client’s treatment will vary according to their individualized treatment plan. Additional sessions will be scheduled as needed. 

C4PW’s treatment team will conduct a comprehensive assessment and determine the client’s placement needs. Once the client has been placed in the appropriate level of care, they will meet with their counselor to discuss and design an effective, individualized treatment plan.

Families are included in client’s treatment from the moment of admission.  C4PW offers two monthly multi-family sessions and a minimum of two individual family monthly sessions. Additional sessions will be scheduled as needed. 

Yes, C4PW’s Medical Director is a Psychiatrist.

Facility FAQ

C4PW has an area with fitness equipment, an art studio, a basketball court, and a recreation area with games.

Clients are allowed to use their devices during down time. Devices must be silenced and are not to be used during group or individual sessions.

Yes, C4PW has a spacious recreation/downtime area with a living room area, air hockey and pool tables.
